Stapleton Physical Therapy
Opens New Facility

The much anticipated relocation of the Stapleton Physical Therapy Group (SPTG) has finally happened! Please join us in welcoming them to "our side of the street" and into their wonderful new home within our medical complex. Their new address is: 6900 E. 47th Avenue Drive, Suite 150. They have a separate, private entrance or can be accessed through an internal doorway which connects our suites.

We feel that this move will have a significant impact on our ability to maximize the effectiveness of the treatment plans for injuries requiring physical therapy. As this move furthers our ability to provide comprehensive medical care, we have devoted most of this newsletter to introducing you to their staff and services.

Stapleton Physical Therapy is affiliated with MATRIX Rehabilitation, an extensive national network of physical therapy providers. Although we are separate entities, this "team" approach has played an instrumental part in rehabilitating our Workers’ Compensation injuries since 1987.

In addition to their new facility in our building, they have 15 other locations throughout the Denver metro area.

Services within the Matrix network include:

  • Physical & Occupational Therapy
  • Hand Therapy & Splint Fabrication
  • Ultrasound & Bio-feedback
  • Orthotics
  • Job Site Evaluations
  • Therapeutic Procedures
  • Therapeutic Exercises
  • Pre-Employment Testing
  • Functional Capacity Evaluations
  • Massage Therapy, and more…

Their talented staff of on-site caregivers include:

Carolyn West, Physical Therapist

Carolyn graduated from the University of North Dakota with a B.S. in Physical Therapy. She has 14 years experience, including acute hospital, outpatient (including neuro), orthopedic and workers’ compensation settings in Chicago and Denver. She has additional training in manual therapy and McKensie training, and is WorkSTEPS certified for post offer testing and modified functional capacity evaluations.

Nicki Venn, Physical Therapist Assistant

Nicki graduated from Professional Skills Institute in Toledo, OH with an A.A.S. as a Physical Therapist Assistant. She has 2 years experience focusing on outpatient, aquatic, and workers’ compensation settings.

Nancy McElroy, Office Coordinator

Nancy has worked for the company since 1994 as the Office Coordinator and Physical Therapist Assistant. With 6 years of experience in the workers’ compensation setting, she can help you with questions regarding any of their services.

Audie Poole, Certified Massage Therapist

Audie received his massage certification from the Colorado School of Healing Arts, and has an undergraduate degree in Exercise Science. Audie has been an employee of MATRIX for 4 years, specializing in Swedish, Deep Tissue, and Neuromuscular massage techniques.

Information on Updated Forms

In an effort to streamline and simplify the paperwork generated with workers’ compensation injuries, we have re-designed our Work Status Report and Treatment Authorization Form. This re-design was prompted by phone calls and suggestions from a number of our clients who were kind enough to share their input with us. You will notice several changes on each form, and we hope you will find them easier to read and understand than the previous versions.

If you are NOT using Treatment Authorization Forms that look like the one pictured at left, you are living in the past! PLEASE let us know so we may send updated forms to you. There is quite a bit more information and options on the new version.

Also, note that at the bottom of the new Work Status Report there is an area that the patient must read and sign before leaving our office, indicating that they "have received a copy of, and understand" the report. This should help eliminate any misunderstandings regarding the patients’ limitations and restrictions, especially as it relates to activities away-from-work.

Pinnacol Accreditation

We are happy to announce that Pinnacol Assurance (Colorado’s largest underwriter of workers’ compensation insurance) has designated our clinic as a preferred provider of industrial medicine, and accepted us into their "Select-Net" group of providers. We are honored to receive this accreditation, and are excited about building a lasting relationship with Pinnacol and their policyholders.

Thank you Pinnacol,
we appreciate your confidence in our team!
